This site is for the Waiver for all AK Volleyball and High 5 Tutoring LLC sessions, lessons, and other events.
All Participants must sign this waiver below before attending any AK Volleyball/High 5 Tutoring LLC sessions, lessons, and other events. Each participant must sign this waiver prior to the event or participation will be denied.

I am fully aware of the risks of participating in one of these events. I ACKNOWLEDGE THAT BY SIGNING THIS DOCUMENT, I AM ASSUMING THE RISKS, AND AGREEING TO INDEMNIFY, NOT TO SUE AND RELEASE FROM LIABILITY THE ORGANIZER OF THIS EVENT, ANGELA KNOPF-CASTLE, HIGH 5 TUTORING LLC, AND THEIR RESPECTIVE AGENTS, EMPLOYEES, VOLUNTEERS,
I am aware of the situation with COVID-19 and other illnesses and understand the added risks involved to participate in this event.
Angela has a medically fragile newborn so please do not come if you are sick. If my athlete has any slight symptoms (ie, cough, fever) I will stay at home, and notify Angela immediately. Please do not send a sick athlete to any session.
